In order for any student to PRACTICE and PARTICIPATE in sports they must have the following

1. Physical - Download forms here and take them to your doctor. Return them to our school. If you had a physical last year you do not have to do it again.

2. Medical History - Fill out form online here.

3. Drug Testing Consent Form - Fill out form online here. 

4. UIL General Information Form - Fill out form online here.
